 myLaurel Health has three clinical service programs: 

  • Rapid Advanced Care: on-demand in-home acute care
  • Acute Care at Home: hospital patients are discharged home, and advanced care is continued in the home while simultaneously providing transition care, reducing time in the hospital and significantly reducing readmissions 
  • Recovery at Home: post-hospital discharge program  

Summary Description
We are seeking a dynamic and results-oriented Senior Healthcare Liaison, RN to spearhead the launch and growth of a new service line within a prominent hospital system. The ideal candidate will possess a strong clinical background, coupled with exceptional relationship-building and strategic thinking abilities. This role requires a collaborative individual who can seamlessly integrate into the hospital’s culture while fostering strong partnerships and expanding our service line.

 

Key Responsibilities

  • Relationship Building: Cultivate and maintain strong partnerships with hospital leadership, medical staff, and key stakeholders to foster collaboration and support. 
  • Patient Referrals: Proactively identify and develop referral sources within the hospital and community to drive patient volume.
  • Patient Engagement: Educate patients/caregivers, ensuring understanding and agreement to the program, providing discharge instructions, and visit expectations.
  • Strategic Development: Partner closely with Client Success to execute a comprehensive growth strategy for the new service line, aligning with the hospital's overall objectives.
  • Clinical Expertise: Leverage clinical knowledge to provide expert guidance and support to the healthcare team in advocating and recommending patients for the program.
  • Operational Integration: Collaborate with hospital staff to ensure seamless integration of the new service line into existing workflows and systems.
  • Performance Measurement: Track and analyze key performance indicators to measure the success of the service line and identify areas for improvement.
  • Team Collaboration: Work closely with the myLaurel team to ensure alignment with company goals and strategies.

Typical Day in the Life:
 

  • Attend daily hospital round huddles
  • Census and chart reviews
  • Referral tracking
  • Hospital Staff & Provider program education and engagement
  • Patient/Caregiver program education and engagement
  • Attend other meetings as needed

Requirements

  • Current and unrestricted RN
  • Minimum of 5 years of clinical experience in an inpatient or emergency room hospital setting
  • Proven track record of success in developing and launching new healthcare services.
  • Strong analytical and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
 

Preferred Qualifications

  • Bachelor's degree in Nursing or related healthcare field required

Special Considerations

  • Prolonged periods of sitting at a desk and working on a computer.
  • Walking and standing for a period of time

Location:  On-site

Compensation & Benefits:

  • Hourly rate: $50 per hour; plus productivity bonus based on performance metrics.
