myLaurel is hiring passionate Physician Assistants (PAs) to provide integrated medical services in patients' homes in the Greater New York City area. myLaurel is a leader in home-based acute care for frail, elderly, and complex patients to avoid ER visits, hospital admissions, and post-acute care stays.
Pay Rate: This role will be compensated at a rate of $149,000 to $175,000 annually with a sign-on bonus.

As a member of the Clinical Delivery team, you will be a part of a team that is bringing patient care directly to the home, while ultimately improving outcomes, patient experience and reducing overall cost of care. We are looking for motivated, creative individuals to join our team and help us create a new standard of care!
In this role you will work 36-40 hours scheduled per week, with some weekends and holidays included. Shifts will be 8-10 hours, and we will consider part-time availability.
Key Responsibilities:
- Perform in-home problem-focused assessments including evaluation of past medical history, review of systems, health screenings, medication reconciliation, vital signs, and focused physical exam
- Identify urgent and emergent situations and intervene appropriately
- Order, perform, and analyze diagnostic tests (e.g., labs, EKGs, ultrasound).
- Provide therapeutic interventions (e.g., IV fluids, IV/IM/PO medications),
- Coordinate testing and follow-ups.
- Integrate patient goals into care plans, including palliative principles.
- Educate patient on topics such as disease process, medication, and compliance
- Communicate with referring clinicians and ensure transition of care post-visit.
- Support clinical guidelines, pathways, and quality initiatives.

Requirements
- Current State Licensure as a PA in New York.
- Minimum two years of experience in relevant areas (e.g., ED, Urgent Care, Hospital).
- Current BLS certification (ACLS preferred).
- Physically ability to navigate stairs and complete patient care in home settings.
- Eligibility to work in the U.S. without sponsorship.
