🌟 Summary

The Care Coordinator plays a crucial role in ensuring our care teams are deployed effectively, resources are optimized, and communication flows seamlessly between field teams, telehealth providers, and patients. You’ll manage day-to-day logistics, coordinate patient and clinician schedules, and be the hub that keeps operations running smoothly for acute-level care delivery.


📋 Key Responsibilities

  • 🚑 Coordinate deployment of field care teams across multiple markets based on patient needs and clinician availability.
     
  • 📅 Manage daily logistics: patient scheduling, resource planning, and operational troubleshooting.
     
  • 💬 Serve as the communication hub between field teams, telehealth providers, and patients.
     
  • ⚠️ Monitor & respond to field safety alerts, escalating issues in real time.
     
  • 💻 Leverage multiple software platforms to perform duties efficiently.
     
  • 📞 Assist with overflow inbound communications from patients or internal teams.
     
  • 📈 Support process improvements and quality initiatives for care delivery.
     
  • 🗣 Communicate clearly and effectively with patients and providers.

✅ Requirements

  • 2+ years of experience in a healthcare setting (ambulatory, home health, pre-hospital, etc.)
     
  • 📝 Strong documentation and communication skills
     
  • 🌐 Remote-ready with ability to work independently and collaboratively
     
  • 🚨 Proven ability to escalate care or operational issues appropriately
     
  • 🧠 Calm under pressure with a strong sense of urgency
