myLaurel is seeking an adaptable Clinical Liaison (RN, Paramedic, or LCSW) for a high-impact, hybrid role in the Philadelphia area. As a Clinical Liaison - Floater, you serve as the critical "connective tissue" for our hospital operations, acting as an elite, flexible resource to ensure program continuity and patient access to our innovative Acute Care at Home model.
Compensation & Role Details
Pay Rate: $50.00 per hour.
Contract Flexibility: 1099 Contract with a minimum of 8 hours per month (potential for increase based on census).
Hybrid Model: Combines on-site hospital presence across Philadelphia with remote administrative flexibility.
Impact: Directly facilitate care transitions that keep vulnerable patients out of the hospital and in the comfort of their homes.
Core Responsibilities
Patient Identification: Perform high-acuity Clinical Chart Reviews and census monitoring to identify patients qualifying for acute care at home.
Referral Management: Build and maintain strong Physician Relations and coordinate with hospital leadership to ensure steady referral volume.
Program Continuity: Provide expert gap coverage for our core Clinical Liaisons during absences, PTO, or new facility launches.
Clinical Documentation: Ensure precise data entry within EMR systems to maintain HIPAA compliance and operational tracking.
Provider Education: Serve as a subject matter expert on myLaurel services, educating medical staff on Medical Necessity and program benefits.
Required Qualifications
Active Licensure: Current, unrestricted license as a Registered Nurse (RN), Licensed Social Worker (LSW/LCSW), or Paramedic in the State of Pennsylvania.
Experience: Minimum of 2 years in Emergency Medicine, ICU, Case Management, or Inpatient Acute Care (4+ years preferred).
Technical Skills: Proficiency with EMR platforms and remote collaboration tools.
Mobility: Valid driver’s license and reliable transportation for frequent travel across the Philadelphia metro area.
Adaptability: Proven ability to build rapport quickly and integrate into various hospital cultures.
